How is a Prostate Biopsy Performed?

There are two main methods to perform a prostate biopsy: transperineal (TPUS or TPB) biopsy and transrectal (TRUS) biopsy. Both procedures are typically done under general anesthesia to ensure patient comfort.

  • Transperineal (TPUS or TPB) Biopsy: In a transperineal biopsy, a small ultrasound probe is inserted into the rectum. The probe generates images of the prostate, which are displayed on a screen, as these images guide the doctor in precisely inserting a needle through the skin between the anus and the scrotum (the perineum) to collect tissue samples. This method allows for targeted sampling of the prostate, reducing the risk of missing cancerous areas. It is particularly useful for patients with suspected cancer that was not detected in previous biopsies or those with difficult-to-reach prostate regions.
  • Transrectal (TRUS) Biopsy: In a transrectal biopsy, the needle is directly inserted into the prostate through the rectum. Similar to the transperineal method, an ultrasound probe is used to create images of the prostate, guiding the needle to the precise locations for tissue sampling. This method is commonly used and is known for its efficiency and shorter procedure time.

What to Expect from a Prostate Biopsy Procedure

A prostate biopsy is a medical procedure used to diagnose prostate abnormalities, such as cancer. The procedure involves collecting small samples of tissue from the prostate gland for microscopic examination. Here’s a step-by-step overview of how a prostate biopsy is typically performed:

Before the biopsy, your doctor will review your medical history, discuss symptoms, and explain the biopsy process. As part of prostate biopsy preparation, you may need to stop certain medications and take antibiotics to prevent infection after the prostate biopsy procedure. Fasting may also be required for a few hours before the procedure. This consultation ensures you understand the procedure and can ask any questions.

To minimize discomfort, a local anesthetic is applied to numb the area around the prostate. You will then be positioned either on your side with knees drawn up to your chest or on your back with legs raised in stirrups, providing optimal access to the prostate gland.

An ultrasound probe is inserted into the rectum to create images of the prostate. Using these images, the doctor guides a thin, hollow needle through the rectal wall into the prostate gland.
